    What are your clinic hours?

    A:

    Monday – Thursday from 7am to 4:30pm and Friday from 7am to 12pm.

  • Q:

    How can I set up an appointment with The Cypress Clinic?

    A:

    Feel free to call our office during regular hours to schedule an appointment. You will be asked about the nature of your visit to properly schedule enough time for your appointment.

  • Q:

    It’s my first visit the The Cypress Clinic, do I need my medical records from my previous doctors?

    A:

    If you have seen another physician, it is helpful to have your previous medical records, but not a requirement. A consent for release form must be signed should you choose to share this information with us. We will forward this form to your previous physician.

  • Q:

    I have to reschedule / cancel my appointment. What do I do?

    A:

    Please notify our office within 24 hours by telephone. This courtesy will allow other patients to utilize that time.

  • Q:

    What do I do if I need after hours care?

    A:

    If an emergency occurs after our regular hours, please dial 9-1-1 or go to the nearest medical emergency room. If you call our office outside of regular hours, our answering service will respond and contact the doctor on call at 337-527-2491.

    How can I get a question to my medical provider?

    A:

    You are encouraged to call us with any medical questions or concerns. The office staff or physician will contact you in a timely manner.

  • Q:

    Do you need my insurance information at the time of my visit?

    A:

    Yes. The Cypress Clinic requires your insurance card at the time of service. We will file all claims to our participating insurance groups. Payment of insurance co-pays, deductibles, and all fees NOT covered by your insurance provider are expected at the time of service. Fees may be paid by cash, check, Visa, Mastercard, and Discover.

  • Q:

    I need a prescription filled. What do I do?

    A:

    The Cypress Clinic encourages you to refill all medications during a regular office visit. If this is not possible, please contact your pharmacy and they will notify us via fax. Please allow 3 business days for all refills.
